Getting Started as a client with Aysana Health

Good news! We are accepting new clients in both locations (Grove and Owasso). To set up your first appointment give us a call at the office you would like to visit and schedule your visit. Before coming to your first appointment, we recommend downloading our Client Intake Form and completing that prior to your visit (see below).

Client Evaluation

One of the ways Dr. Rochelle will evaluate a client is to use a system called EAV (Avatar).

Another system Dr. Rochelle uses to evaluate her clients is a system called Electro Interstial Scan.

New Client Forms

Please print these forms out and have them ready for your appointment with Dr. Rochelle. If you have lab work or test results, please email them prior to your appointment. This allows the doctor to analyze data prior to your scheduled appointment and ensures more time for your consultation and evaluation. Keep in mind that she is not in her main office on Wednesdays or Thursdays, so if you have an appointment on Friday and email papers in on one of those days she still won’t see it until Friday.
